Are you an OT or COTA ready to see the part of your patient’s recovery a clinic never shows you? Home-based therapy gives you the full picture — how your patient actually navigates their kitchen, their stairs, their bathroom, the everyday tasks that matter most to their independence. That context changes everything, and so do outcomes.

Empress Healthcare Inc., a Medicare-certified home health agency, is expanding its therapy team in Orange County. We’re looking for licensed Occupational Therapists and Certified Occupational Therapy Assistants committed to function-focused, patient-centered care in a home setting.

The work

What you’ll do

Occupational Therapist

OT

  • Complete each patient’s initial evaluation and build their plan of care and goals — then keep it current with reassessments every 30 days
  • Deliver hands-on, physician-ordered occupational therapy in patients’ homes across Orange County
  • Help patients rebuild the everyday skills that matter most to them — fine motor movement, self-care, and the hands-on tasks of daily living
  • Check in with physicians on how each patient is progressing, and adjust the plan of care as needed
  • Write progress notes each visit, complete recertifications, and wrap up each case with a discharge summary
  • Teach patients and family members the home programs and fine-motor exercises that keep progress going between visits
  • Mentor and guide the COTAs and Home Health Aides working alongside you, including regular check-in visits
  • Join case conferences, in-service trainings, and team conversations about how we’re doing as a group
Certified Occupational Therapy Assistant

COTA

  • Carry out the hands-on therapy plan your supervising OT has built for each patient
  • Help patients work toward their goals — fine motor skills, self-care, and everyday independence
  • Keep your supervising OT in the loop on how each patient is doing, and flag anything that needs a second look
  • Teach patients and families the home programs and fine-motor exercises that are part of their plan
  • Keep clear, timely notes, and help your supervising OT prepare progress updates
  • Take part in case conferences, in-services, and team conversations right alongside everyone else

Both roles work hand-in-hand with Empress’s skilled nursing and case management team on every patient’s plan of care — this is never a solo job.

The team

Who you’ll work with

We keep things close-knit and easy to navigate — you’ll always know who to call:

  • OTs work closely with our Clinical Manager and Director of Clinical Services. Think of them as your go-to people for support, questions, and mentoring — not distant supervisors.
  • COTAs partner day-to-day with their supervising OT, with the same Clinical Manager and Director of Clinical Services in their corner too.
  • Everyone on the therapy team works side-by-side with our nursing staff and case managers — good home health care is a team effort, not a solo act.

And at the end of the day, every one of us — from the newest COTA to the Director of Clinical Services — answers to the same person: the patient in front of us.

Requirements

What we’re looking for

  • Current, active California license or certification as an Occupational Therapist or Certified Occupational Therapy Assistant, in good standing
  • Graduate of an accredited Occupational Therapy (or Occupational Therapy Assistant) education program, with national certification where applicable
  • Two years of home health or clinical experience is a plus, but we welcome strong candidates at every experience level
  • A patient-centered approach to care and comfort working independently in a home setting
  • Willingness to travel within Orange County for in-home visits

Bilingual candidates are strongly encouraged to apply — particularly Spanish, Armenian, Korean, Tagalog and Chinese speakers.
