LVN

• Provide skilled nursing care ordered by the physician and carry out each patient’s plan of care, under the supervision of the RN
• Accept, write, and execute physician orders — co-signed by the RN
• Administer medications and treatments as ordered
• Watch for changes in each patient’s condition and report your findings to the RN and physician
• Teach and counsel patients and their families on the nursing care needs and related concerns of care at home
• Keep accurate, complete nursing records, documenting your actions and interventions in the visit report
• Evaluate with your supervising RN how each patient’s care is progressing
• Join staff meetings, patient care conferences, and in-services — and help involve the right team members, such as PT or SLP, when a patient needs them
• Take on-call duty on a rotating basis — nights, weekends, and holidays
